Single Sample COA
To use this COA template, please install the bika.coa add-on. Single sample COAPDF example. Multi sample COA
The COA is laid out in different sections: HeaderUnique COA IDs. More than one COA may be issued for the same Sample, say for provisional publication of early results and republication when all results are available. For this reason Bika COAs have unique IDs to refer to them correctly. The ID is prominently displayed in the COA header COA IDs are generated by including a sequential postfix to the Sample ID and prefixing it with the clients ID for lab staff to easily reference the COA's sample The rest of the header is laid out business letter style, the lab's logo and If it is accredited, their accreditation logo are displayed in the top right corner of the document. Per regulation the accreditation logo is smaller than the lab logo The Client address references the Client Contact who requested the Analyses and to whom the COA is mailed Sample SummaryAll the Sample Identification, descriptive attributes and transition dates are listed here The Sample and Batch IDS are hyperlinked through to the LIMS, and the client may look them up online (after logging on)
